For new clients, once we have spoken and scheduled your first session, please complete the following steps:
1. After we have agreed upon a time for our initial meeting, please log into the patient portal in order to register. You will be asked to read and electronically sign several documents, as well as provide information about yourself (or your child) and your insurance. Please note that the background information form must be completed in one sitting.
2. Please bring a valid photo id and your insurance card to your appointment.
3. If you choose to engage in teletherapy, I will send you an invitation near the start time of our session.
A copy of the HIPAA Notice of Privacy Practices is available for you at the office.
Dr. Gutmann will discuss the forms and answer any questions you may have about them during the initial meeting.
